The Palliative Care Community Team (PCCT) is a centralized service that links patients and families to palliative care services across Durham Region. The PCCT includes Patient Navigators, Palliative Care Coordinators and Supportive Care Counsellors. Working in partnership with community partners, our PCCT provides clients and their families with holistic health care including emotional, social, and spiritual support. The PCCT is available for anyone diagnosed with a life-limiting illness at any time from initial diagnosis through bereavement.
Our Patient Navigators are Registered Nurses who are clinical experts with a unique role in patient care. They are members of the healthcare team who journey with individuals and families with a life-limiting illness through advocacy, and pain and symptom management; understanding and deciding goals of care and providing knowledge and resources to navigate the health care system. VON Durham Patient Navigators will work within the community to provide care in a timely manner.
VON Patient Navigators assess clients using a holistic model based on physical, spiritual, emotional and social needs.
Patient Navigators can provide information, referrals, and advocacy to assist individuals living with life-limiting illness to stay at home for as long as they may choose. We can provide information regarding: community-based supports, palliative care referrals, and advance care planning. Following an initial assessment, the Coordinator may offer supports including: ongoing telephone contact, a volunteer to provide respite care, assistance with children and youth in the home and/or visit with the caregiver to listen, share, and provide information.
Palliative Care Coordinator
Following an initial assessment, the Palliative Care Coordinators will discuss the support(s) that are available and you decide when and how you would like to access our services.
A person with a life-limiting illness may face pain, anger, and fear, not to mention a multitude of losses, including: work, social activities, physical abilities, or sense of self. The Palliative Care Coordinators strive to achieve holistic hospice Palliative Care by providing emotional, social, spiritual, physical, and practical support to individuals facing a life-threatening illness. We believe that you and your family are at the center of care planning, which means you are in the ‘driver’s seat’. We help to maintain your dignity and personal choice – YOU direct your care needs.
Supportive Care Counsellor
VON Durham has Social Workers and Psychotherapists who provide support and counsel. Having a life-limiting illness can result in many difficulties for individuals, families, and caregivers. Our Supportive Care Counsellor provides professional counselling for the unique emotional, social, and spiritual issues that may arise.
