WIN provides expert Mental Health Case Management services to individuals requiring ongoing support for a chronic mental health illness. Clients receive individualized ongoing assessments, interventions, treatments, counselling and referrals, from mental health nurses.

VON Mental Health nurses work with clients to reach an optimum level of functioning and independence within the community. 

WIN Services

The WIN program promotes and facilitates independent living.

  • Skillful assessment of mental health
  • Monitor and address any changes in client behavior or thinking
  • Supportive counseling that is knowledgeable and non-judgmental
  • Advocacy; ensuring goals are met
  • Enrichment through encouraging, fulfilling, and satisfying activities
  • Medication monitoring and health education
  • Rehabilitation skills and support
  • Supports with budgeting for food and housing if needed
  • Links to other resources and support services through referral

Program Benefits

This service is provided by specially trained mental health nurses to clients requiring long-term support. As VON is accredited through Accreditation Canada, you can depend on the highest quality care from Canada’s oldest and most respected home and community support services provider.

  • Case management plan builds on the strengths, abilities, and personal goals of the client
  • Focus on setting specific, realistic goals that will improve quality of life
  • Provides stability and guidance
  • Nurses work closely with other health professionals to coordinate care
  • Financially supported by the Central East LHIN with no cost to clients

Participant Eligibility

  • Resident of Durham Region
  • 16 years of age or older
  • Willing to work with a nurse from the VON Mental Health Team
  • Assessed as having a severe mental health diagnosis requiring ongoing support
  • Experiencing difficulty with daily functioning
  • Must have control over own behavior for reasons of safety
  • Clients must be able to ensure that they will not exhibit violent or aggressive actions toward themselves or others

Client Referral

WIN Mental Health Services can be accessed by referral from any of the following:

  • Hospitals and clinics
  • Community mental health agencies
  • Substance abuse programs
  • Family physicians
  • Psychiatrists and specialists
